Job Description:
RN Patient Care Coordinator Join our growing team of dedicated heatlh care professionals! Caris HealthCare is seeking an experienced RN Patient Care Coordinator for Lenoir City and surrounding communities. The Patient Care Coordinator (PCC) coordinates all aspects of patient/family care and support, also facilitates communication and interaction among personnel providing services to terminally ill patients. PCC is a RN Supervisor who oversees our clinical staff and coordinates compassionate end of life care for our hospice patients and their families. Must be willing to provide direct patient care. Education, training, experience:
At least 2 years of supervisory experience in a HealthCare setting is preferred. Tennessee RN license, required. Computer skills are a must.
